What is Teeth Whitening

Teeth whitening is a cosmetic dental procedure aimed at lightening the color of your teeth. It can significantly enhance your smile and boost your confidence. Teeth can become discolored for various reasons, including aging, consumption of certain foods and beverages (like coffee, tea, and red wine), smoking, and poor dental hygiene.

Causes of Tooth Discoloration

Tooth discoloration can be caused by a variety of internal and external factors, which can be summarized as follows:

Extrinsic Factors
These affect the surface of the teeth and cause staining on the enamel.
  1. Dietary Habits
Pigments in dark-colored beverages like coffee, tea, red wine, and cola can stain the tooth surface.
  1. Smoking
Nicotine and tar in tobacco can cause teeth to turn yellow or brown.
  1. Poor Oral Hygiene
Inadequate brushing or incorrect brushing techniques can lead to plaque and tartar buildup, causing discoloration.
  1. Certain Mouthwashes
Mouthwashes containing chlorhexidine can cause surface staining of teeth.

Intrinsic Factors
These affect the inner structure of the tooth (dentin) or enamel formation.
  1. Aging
    As we age, the enamel wears down, revealing the yellowish dentin underneath, making teeth appear more yellow. 
  2. Medication Effects
Certain medications, such as tetracycline antibiotics, can cause internal tooth discoloration, especially when taken during childhood.
  1. Excessive Fluoride
Excessive fluoride intake (e.g., high fluoride levels in drinking water) can cause fluorosis, leading to white or brown spots on teeth.
  1. Trauma
Teeth may discolor after trauma due to internal bleeding or nerve damage.
  1. Disease or Treatment
Certain diseases or treatments (e.g., chemotherapy) can affect tooth color.
  1. Genetic Factors
Some people are born with thinner enamel or naturally darker teeth, which can make teeth appear more yellow.

Methods of Teeth Whitening

There are several methods available for teeth whitening, each with its own advantages:

1. In-Office Whitening: This is a professional treatment performed by a dentist. High-concentration hydrogen peroxide or carbamide peroxide is used as the bleaching agent to break down surface stains through an oxidation process, along with a Blue light (typically with a wavelength of 480-520 nm) to accelerate the chemical reaction of the bleaching agent, enhancing the whitening effect. In-office blue light teeth whitening is a fast, safe, and effective method for significantly improving tooth color in a short time.

Procedure
1. Examination and Cleaning:
The dentist will first examine the health of your teeth and gums, followed by a professional cleaning to remove plaque and tartar.
2. Gum Protection:
A protective gel or rubber dam is applied to the gums to prevent the bleaching agent from irritating the soft tissues.
3. Application of Bleaching Gel:
The high-concentration bleaching gel is evenly applied to the surface of the teeth.
4. Blue Light Exposure:
The teeth are exposed to blue light for about 10-15 minutes per session, usually repeated 2-3 times, depending on the condition of the teeth.
5. Rinsing and Evaluation:
The bleaching gel is removed, the teeth are rinsed, and the whitening results are evaluated.

2. At-Home Whitening Kits: Home bleaching is a popular method of whitening teeth that involves using custom-made trays provided by a dentist along with a lower concentration bleaching gel. The gel, containing hydrogen peroxide or carbamide peroxide, is applied to the trays, which are then worn daily for a specified period, allowing the bleaching agent to penetrate the teeth and remove stains. This method is convenient, can be done in the comfort of your home, and is more affordable than in-office treatments. The process typically takes a few weeks to achieve the desired result.
Procedure
1. Consult a Dentist
It is recommended to consult a dentist first to ensure that your teeth and gums are healthy and suitable for whitening.
2. Choose a Product
Select an appropriate whitening product based on your dentist's advice.
3. Custom Trays
Dentists create personalized trays based on the patient's dental structure to ensure even distribution of the whitening agent.
4. Regular Use
Patients use a low-concentration whitening gel provided by the dentist, wearing the trays for a few hours each day over 1-2 weeks, depending on the product instructions and individual circumstances.

● Personalization: Custom trays ensure even application of the whitening agent and minimize irritation.

Considerations Before Whitening

  1. Dental Health
    Ensure you have good dental health with no cavities or gum disease. It’s best to consult with your dentist before starting any whitening treatment.
  2. Sensitivity
    Some people may experience tooth sensitivity during or after whitening treatments. Discuss this with your dentist to find the best method for you.
  3. Color Limitations
    Whitening is effective for natural teeth but may not work on crowns, veneers, or fillings. Discuss your expectations with your dentist.
  4. Variable Results
    Results may be limited for tetracycline-stained teeth or fluorosis

Aftercare

  1. Avoid Staining Foods and Beverages
    For the first 24 to 48 hours after whitening, avoid foods and beverages that can stain teeth, such as coffee, red wine, tea, and berries.
  2. Maintain Oral Hygiene
    Continue to brush your teeth and floss daily to maintain oral hygiene.
  3. Use Gentle Toothpaste
    Choose non-abrasive or sensitivity toothpaste to minimize irritation to your teeth.
  4. Avoid Smoking
    Smoking can cause teeth to stain, so it is advisable to avoid smoking after whitening.
  5. Be Aware of Sensitivity
    Temporary tooth sensitivity may occur after whitening; if symptoms persist, consult your dentist.
  6. Touch-up treatments
    Bleaching can only keep the teeth white for a period of time. Repeated bleaching treatment is needed to keep the teeth white.
